Advance Directive Workshop
Saturday, May 24, 3–5 pm
Join other Viva Village members and volunteers for an afternoon workshop on Advance Care Planning guided by Eriko Onishi, MD, MCR, PhD, a palliative care doctor and OHSU faculty member. Together we’ll explore our wishes, values, and goals for end-of-life planning.
Step-by-step we’ll have the opportunity to review and complete our own Oregon Advanced Directive. Other topics include choosing a Health Care Representative, completing a POLST form, and clearly communicating your wishes to loved ones.
